General Practice in Aged Care Incentive - Webinar Series - Session 3: A conversation with GPs

The Department of Health and Aged Care recently introduced the General Practice in Aged Care Incentive. Eligible providers and practices registered in MyMedicare can receive incentive payments for providing their registered patients living in a residential aged care home with care planning and regular visits, at their home. This incentive responds to the issues raised in the Royal Commission into Aged Care and aims to support more proactive, planned and continuous care for people living in aged care homes from their responsible GP and practice. Peer learning provides an opportunity to explore the incentive and share experiences in its implementation.
You are invited to attend a one-hour webinar to support you in the implementation of the General Practice in Aged Care Incentive. These webinars will include conversations on:
- What this incentive means for you and the potential benefits
- How to utilise the incentive to support high quality, integrated, patient centred care in an aged care setting
- How obstacles involved in operationalising this incentive can be overcome.
This webinar series is being delivered with funding and support from the Australian Government Department of Health and Aged Care.
