The purpose of the Yellow Envelope is to facilitate the seamless transfer of care provided to an aged care resident when they are moving in and out of hospital. The information asked for on the yellow envelope is necessary in the clinical handover of the resident’s care. Using the Yellow Envelope for transfers will provide the resident with safe and continuous provision of quality care.

If you would like to receive a regular supply of Yellow Envelopes (shipments are twice yearly, in February and June), sign up for a subscription and indicate the quantity you’d like to receive.
