Changes to where you can access Referral Templates for General Practice from 1 July 2024

For Practices in Far West NSW:

Practices in Far West NSW can now use HealthLink to send an e-referral. Click here to learn more.

For Practices in Western NSW:

WNSW PHN is investing in better ways to support General Practice so that our communities receive the right care, in the right place, at the right time.

HealthPathways is a web-based health information site which helps clinicians guide patients through the complex local health system.

From 1 July 2024, WNSW PHN will be migrating up-to-date referral templates for Best Practice and Medical Director to the Western NSW HealthPathways site so that you have access to all the resources and information you need in one place.

The webpage you are viewing now will become inactive as of 1 October 2024, when all templates will be accessible through Western NSW HealthPathways.
