Welcome to Chronic Disease Management with CDM Plus!

This one-day face-to-face workshop is suitable for Nurses, Aboriginal Health Workers and Practitioners, Nurse Practitioners, Practice Managers, and GPs looking to improve Chronic Disease Management service delivery.
