WNSW PHN is excited to be offering Primary Health Care Professionals and Practitioners the opportunity to take part in 2 Health Change Methodology (HCM) Courses. Details are as follows:

Who should attend?

  • Any type of front-line service provider: nursing, allied health, medical, mental health, care co-ordinators etc who work in Chronic Disease Management.
  • Senior and junior staff
  • Key program or service designer or decision-makers

The Health Change Methodology courses are designed to:

  • Support clinician confidence and skills working with chronic and complex patients,
  • Building clinician capacity when transitioning to telehealth and mobile technology
  • Provide a consistent way to deliver health literacy and person-centred care –common language and methodology
  • Provide simple tools that work across all levels – clinicians, teams, services and organisations
  • Improve patient engagement and clinical outcomes

Benefits reported from previous course participants include

  • higher patient retention and participation rates,
  • reduced wait lists,
  • lower surgery referrals due to poor patient self-management,
  • better clinical outcomes.

About the Courses:

The foundation course, Person-Centred Practice and Engagement Skills will be conducted online and involves 4 hours of self-directed online modules AND attendance at four weekly Teams Teleconference discussions with 2 time slot options available:

  • Tuesdays: April 11, 18, 25 & May 2 @ 2pm – 3.30 pm
  • Tuesdays: May 2, 9, 16,23, @ 9.30 – 11am

The Workplace Application Course will be held a few months later to give you time to digest the information and implement any practice changes you choose to make.

It is a one day, face to face course (Face to Face locations are Bourke and Broken Hill) and is focused on application, trouble shooting and case discussion. There is also an option for a teleconference session if you are unable to attend the face to face workshop.
