Marking World Diabetes Day with initiatives to support Type 2 Diabetes care and awareness through the CP-D program

The Far West and Western NSW region is supporting World Diabetes Day on 14 November. The Care Partnership – Diabetes (CP-D) program partners are launching a range of initiatives to support people living with type 2 diabetes and the health professionals who provide their care. These include:

  • The final two ECHO sessions as part of the 2024 Spring/Summer Series are scheduled for today, 14 November, and 12 December (click the links to learn more and register). These sessions support clinicians’ access to evidence-based, multidisciplinary information for their patients by discussing patient cases with a panel of experts and as well as peers.
  • The ‘Far West of Centre’ podcast (available on your podcast app/site) available from Monday 11 November featuring a special edition about type 2 diabetes and living in Far West NSW.
  • A Western NSW social media campaign to raise community awareness about type 2 diabetes and offer advice from people with lived experience and diabetes experts. Keep an eye out for Western NSW LHD’s Facebook and Instagram’.
  • A new Type 2 Diabetes Case Conferencing service that connects people living with diabetes in Western NSW, their GP, nurse and allied health practitioners to expert advice from endocrinologists.
  • A Type 2 Diabetes Community of Practice discussion group for health professionals interested in diabetes in Western and Far West NSW and connecting with colleagues. For World Diabetes Day, anyone who posts in the chat group between 11 and 24 November 2024 will go into a draw to win one of three $50 Iconic vouchers!

The CP-D program is supported by a partnership comprising Far West LHD, Western NSW LHD, Western NSW PHN and RDN, and is funded by the NSW Ministry of Health. More information about the program is available here.
