Care finder

Care finders assist eligible older people who are unable to link into aged care supports and/or community supports. 

Care finder supports older people to access aged care services, health services and community supports throughout Western and Far West NSW.

Designed for

Older people requiring extra, intensive support to access aged care services and other supports in the community. To receive care finder support, a person must:

  • Have no carer or support person who can help them; or
  • Not have a carer or support person they feel comfortable or trust to support them; and
  • Be eligible for government-funded aged care.

Service Region

Care finders are located across the whole WNSW PHN region and will meet people to provide the service face-to-face whenever possible. Please contact the Care Finder service located in your region.
