Embedding pharmacists to strengthen care in aged care homes

The Aged Care On-site Pharmacist (ACOP) program provides funding for residential aged care homes (RACHs) to include a credentialed pharmacist as part of their clinical care team.

The pharmacist may be employed directly by the home or engaged through a community pharmacy, working alongside staff to support safer, more effective use of medicines.

The program was introduced following recommendations from the Royal Commission into Aged Care Quality and Safety.

How the program works

An on-site pharmacist works within the aged care home to improve health outcomes and provide ongoing support to staff and residents.

They work closely with:

  • Nursing staff and management
  • General practitioners
  • Allied health professionals
  • Residents, families and carers

Participation in the program is voluntary for aged care homes.

What an on-site pharmacist can support

An on-site pharmacist is part of your team and can help:

  • Optimise medicines in collaboration with residents, families and care teams
  • Identify and resolve medication-related issues
  • Conduct personalised medication reviews
  • Support Quality Use of Medicines (QUM) activities and accreditation
  • Contribute to Medication Advisory Committee (MAC) meetings
  • Improve medication storage and handling practices
  • Support safe transitions of care

ACOP program or RMMR program

The ACOP program is different from the Residential Medication Management Review (RMMR) and QUM program.

Aged care homes can access pharmacist services through one program only, not both.

RMMR and QUM services

  • Delivered by visiting pharmacists
  • Provide medication reviews at set intervals or when needed

ACOP program

  • Pharmacist is based within the aged care home
  • Provides ongoing, day-to-day support
  • Works as part of the care team
