Western NSW Primary Health Network (WNSW PHN) commissions a wide range of health services to improve access to care and respond to the evolving needs of our communities. We fund and support local primary health services, programs and initiatives across Western and Far West NSW to address gaps and ensure people receive the care they need.

Our commissioning model ensures that primary health services in our region are planned, funded and evaluated in partnership with service providers. We aim to address service gaps and deliver better outcomes by investing in programs that are evidence-informed, inclusive and community-driven. While we do not directly deliver care, we play a critical role in funding local health services that meet priority needs.

Data collection and consent

All PHN-commissioned mental health and suicide prevention services funded by the Department of Health, Disability and Aged Care are required to collect the Primary Mental Health Care Minimum Data Set (PMHC-MDS).

WNSW PHN collects this data on behalf of the Department to support service planning and quality improvement. Consent to share this data is optional and your privacy is always respected.

For service providers, partnering with WNSW PHN through commissioned funding offers the opportunity to deliver targeted, evidence-informed care. We work closely with providers across Western and Far West NSW to design services that respond to local health needs and support stronger community outcomes.
