If you’re one of our commissioned service providers, this page contains important information, forms and systems that support your partnership with Western NSW Primary Health Network (WNSW PHN).
From reporting requirements to secure payment processes, our goal is to make it easier for you to meet compliance obligations while delivering high-quality care to the communities we serve.

Service provider reportable incident form

As part of our commitment to strong health partnerships, WNSW PHN requires all commissioned service providers to report specific incidents under their service agreements.
If you deliver services on our behalf, it’s essential to report any of the following through our official process:
- Critical incidents
- Notifiable complaints
- Notifiable incidents (as defined under Work Health and Safety legislation)

This process helps us work together to deliver safe, high-quality services while meeting compliance and duty-of-care obligations across all funded activities.

Eftsure software solution

To strengthen our financial systems, WNSW PHN has introduced Eftsure, a secure software solution that helps verify vendor information before payments are made.
If you’re a new provider or vendor in our system, you’ll receive an email or phone call from Eftsure (via verifications@wnswphn.org.au) requesting confirmation of key business details, including:
- Business name
- ABN
- Bank account information
This extra step supports secure and accurate payments as part of our commitment to transparent operations.
