With our region’s growing demand for quality care, Western NSW Primary Health Network (WNSW PHN) understands the importance of having robust after hours plans in place. Residential aged care after hours plans ensure that the aged care providers have documented the necessary support and guidance to continue delivering exceptional care outside of regular business hours.
WNSW PHN has partnered with MediCoach, who specialise in providing tailored training solutions, to support RACHs in the development of your localised after hours plans. MediCoach’s expertise in improving healthcare team performance equips staff with the tools they need to enhance their capability to provide seamless, high-quality care at all times, including after hours.
Through this partnership, WNSW PHN and MediCoach assist RACH staff in documenting and operationalising local after hours options, ensuring care is not compromised after business hours.

We understand the complexity of providing round-the-clock care, especially in after hours scenarios. WNSW PHN supports RACHs with resources including the after hours plan template, used to document your facility’s after hours care plans.
RACHs in Western and Far West NSW can access after-hours support through Healthdirect’s GP Helpline and Virtual Care Clinic.
