WNSWLHD’s Specialist Intellectual Disability Health Team will discuss evidenced-based approaches to chronic disease management and self-management in people with intellectual disability, and provide a case study example of implementation.

Learning Outcomes

• Apply reasonable adjustments during all stages of managing health conditions in a person with intellectual disability

• Apply evidence-based approaches to management of chronic health conditions in people with intellectual disability

• Apply the principles of collaboration, communication and goal setting to support people with intellectual disability and/or their carers to self-manage their conditions
