This virtual workshop is the first stage in our 12-month Chronic Disease Management and Healthy Ageing Program. It will be delivered over two days, Tuesday 27th and Thursday 29th of July, from 9:30am - 12:30pm.

  • On day 1, we will cover the Aged Care Commission final report, an overview of CDM and HA, optimising health ageing versus managing chronic disease burden, population health and healthy ageing, and strategies and tools supporting CDM and HA.
  • On day 2, explore developing therapeutic relationships for clients with chronic and complex conditions, communication and health literacy, motivational interviewing, goal setting and implementing CDM and HA into practice.

After the workshop, you will have 12 months of online learning, access to a professional community and nurse support to build your knowledge and confidence.
