My Care Future helps people better understand end-of-life care and make informed choices about their future health care. The service provides community education about palliative care and supports people to complete an Advance Care Directive, with guidance from trained peers. This helps individuals, families and communities feel more confident and prepared to plan ahead.
Anyone over the age of 18 who wants to complete an Advance Care Directive, and anyone over the age of 18 who wants to learn more about Palliative and End of Life Care
All of the Western NSW LHD region, with hubs based in Orange and Dubbo
Anyone over the age of 18 can access the service by calling or emailing the Dubbo or Orange office to book in for a workshop, to volunteer or to speak to the coordinator.
