WNSW PHN releases 2024–25 Annual Report

The Western NSW Primary Health Network (WNSW PHN) has released its 2024–25 Annual Report, highlighting continued progress in strengthening primary health care across Western and Far West NSW. The report outlines WNSW PHN’s work to improve equity of access, health system sustainability, and health outcomes for communities in our region.

This year marks 10 years of Primary Health Networks, celebrating a decade of nationally coordinated and locally delivered efforts to build healthier communities.

This year’s report spotlights WNSW PHN’s Innovate Reconciliation Action Plan (RAP), reflecting our ongoing commitment to reconciliation and advancing health outcomes for Aboriginal people and communities.

We are proud to share highlights reflecting progress against strategic goals to engage communities, strengthen health system reform, lead evidence-based commissioning and support high quality, sustainable primary health care.
