How the Keeping Healthy Puura Manti Program Helped Tim Take Control of His Health 

The Western NSW Primary Health Network (WNSW PHN) is proud to support the Keeping Healthy Puura Manti Program (KHPMP), delivered in partnership with the NSW Outback Division of General Practice, Marathon Health, and the Royal Flying Doctor Service. 

In the heart of regional New South Wales, the Keeping Healthy Puura Manti Program (KHPMP) is helping people work towards better health outcomes. Delivered through general practices and Aboriginal Medical Services, the initiative brings together chronic disease practice nurses, Aboriginal health workers, and visiting allied health professionals to support people aged 15 years and over who are managing or at risk of developing two or more chronic illnesses. 

The program’s impact can be seen through individual stories, such as Tim’s experience with the support provided by KHPMP. 

Tim is a 60-year-old client living with Type 2 diabetes. With this condition comes a range of complications, and Tim had been experiencing poor circulation and neuropathy in his feet, placing him at risk of foot ulcers and potential amputation. When a minor foot injury became a slow-healing wound, his GP referred him to a podiatrist through the KHPMP. 

Through the program, Tim received information and guidance on how lifestyle changes could help improve his foot health and overall diabetes management. Together, Tim and the KHPMP clinician developed a plan that included daily foot inspections, custom orthopaedic footwear, and maintaining clean, well-moisturised feet. 

Tim was also supported to make changes to his diet to help manage blood sugar levels and introduced regular low-impact exercise, such as walking and swimming, to support circulation. 

As Tim’s understanding of his condition grew, so did his confidence to self-manage. He achieved improved blood glucose control, which supported wound healing and helped reduce the risk of further ulcers. With ongoing monitoring from his podiatrist, minor issues were addressed early to limit complications. 

Through this coordinated approach, Tim was able to avoid serious health issues such as infection or amputation. 

This story highlights the value of a collaborative, patient-centred approach within KHPMP. It shows how coordinated care can support people managing chronic conditions and contribute to improved health outcomes across the region. 

About the Keeping Healthy Puura Manti Program 

The Keeping Healthy Puura Manti Program (KHPMP) aims to improve chronic disease outcomes across the Western NSW region. KHPMP focuses on three key components: delivering essential clinical and non-clinical services to fill regional gaps; building the capability of general practices and Aboriginal medical services to deliver sustainable chronic disease care; and enhancing system-wide coordination through collaboration with Sub-regional Leadership Groups and Communities of Practice. 

Clients can be referred to the program through their GP or Aboriginal Health Service.
