GP–RACH Connection Hub

Our new GP–RACH Connection Hub makes it easier for Residential Aged Care Homes (RACHs) and General Practitioners (GPs) to connect quickly, efficiently and with clear communication pathways.

Interested practices and RACHs can use the Connection Hub to:

• submit a simple online form to request GP support for aged care residents
• notify WNSW PHN when a GP or practice has availability to take on new RACH residents
• help us match needs and capacity across the region in real time
• stay informed as WNSW PHN reviews submissions and facilitates appropriate connections
