This webinar will provide GPs, NPs and Practice Nurses with a brief overview of the aged care changes from 1 November 2025 with a focus on the and focus on the End-of-Life Pathway. There are several changes to the provision of aged care services that will affect how GPs, NPs and Practice Nurses support patients to access palliative packages of support.

- Identify that ‘Grandparent clauses’ apply and the CHSP extended end date.

- Describe how your next actions differ for patients with no aged care services vs patients with current aged care services.
- Describe and know how to access Compacks palliative supports for younger patients not eligible for aged care services
