This webinar will provide GPs, NPs and Practice Nurses with a brief overview of the aged care changes from 1 November 2025 with a focus on the and focus on the End-of-Life Pathway. There are several changes to the provision of aged care services that will affect how GPs, NPs and Practice Nurses support patients to access palliative packages of support.

Learning outcomes

  • Describe the changes from 4 aged care packages to 6 levels of support@home.
  • Identify that ‘Grandparent clauses’ apply and the CHSP extended end date.
  • Determine eligibility and document for the End-of-Life Pathway packages of support from 1 November 2025.
  • Describe how to access and complete the EOL pathways form and checklist
  • Describe how your next actions differ for patients with no aged care services vs patients with current aged care services.
  • Describe and know how to access Compacks palliative supports for younger patients not eligible for aged care services
