Living with a long-term illness can be hard, especially when it means travelling long distances, seeing lots of different health services, and making big decisions along the way. For Aboriginal and Torres Strait Islander people across Western and Far West NSW, the Integrated Team Care (ITC) program helps make that journey feel more supported and less overwhelming.

How the Keeping Healthy Puura Manti Program Helped Tim Take Control of His Health 

In the heart of regional New South Wales, the Keeping Healthy Puura Manti Program (KHPMP) is helping people work towards better health outcomes. Delivered through general practices and Aboriginal Medical Services, the initiative brings together chronic disease practice nurses, Aboriginal health workers, and visiting allied health professionals to support people aged 15 years and over who are managing or at risk of developing two or more chronic illnesses.
