WNSW PHN Annual Report
Our Annual Report offers a comprehensive overview of WNSW PHN’s achievements, strategic initiatives and financial performance over the past year. It reflects our commitment to improving healthcare access and outcomes through innovation, collaboration and service planning.

The WNSW PHN Strategic Plan outlines our vision, goals and long-term priorities for primary health care in our region. It provides a roadmap for enhancing health equity and addressing local service gaps.

Reconciliation Action Plan (RAP)
Our Innovate RAP outlines meaningful actions to strengthen relationships with Aboriginal and Torres Strait Islander peoples. It promotes cultural respect and guides our role in creating more inclusive and responsive health services.
Needs Assessments

How we plan community health services
Every three years, WNSW PHN conducts a comprehensive Health Needs Assessment using qualitative and quantitative data and insights to identify the region’s key health and service priorities.
These assessments are updated as new data becomes available, ensuring we can plan and commission services that respond to real community needs. Once these needs are identified, Activity Work Plans are developed that outline how we will work with stakeholders and partners to improve health outcomes across our region.

Activity Work Plans outline how WNSW PHN will address the priorities identified in our needs assessments. These plans are developed in consultation with stakeholders and approved by the Department of Health, Disability and Ageing before implementation.
Current approved Activity Work Plans:
Regional Mental Health, AOD & Suicide Prevention Plan

Developed with the Western and Far West NSW Local Health Districts, this two-year plan sets a collaborative direction for mental health, alcohol and other drug support, and suicide prevention in the region. It aligns multiple services and priorities into a unified regional response.

Cultural Safety Framework

WNSW PHN is dedicated to developing culturally safe models of care to improve healthcare access and outcomes for Aboriginal and Torres Strait Islander communities. The Cultural Safety Framework provides primary health care service providers with information and a comprehensive self-assessment evaluation tool to progress towards cultural safety.
WNSW PHN plays a leadership role in advancing these models, ensuring that healthcare services are culturally appropriate and contribute to Closing the Gap in health outcomes between Aboriginal and non-Aboriginal people in Western and Far West NSW. Our Aboriginal Health Council played a key role in the preparation of the Cultural Safety Framework.


Community Engagement Framework
Our Community Engagement Framework outlines a best practice model for involving consumers and communities in health service planning and design. It includes strategies and toolkits to support genuine, ongoing engagement and is relevant to both public and private health services in a range of care settings.

Clinical Governance Framework
This framework guides how WNSW PHN ensures quality, safety, risk management and performance in commissioned services and primary care support. The framework outlines the requirements for service commissioning and continuous improvement in general practice.
