Ensuring the health and wellbeing of residents in residential aged care homes is a 24/7 responsibility. As demand for quality care grows, Western NSW PHN recognises the need for strong after-hours plans to support consistent care outside standard hours.

In partnership with MediCoach, we’re supporting RACHs to develop tailored after-hours plans. MediCoach provides practical training to strengthen team capability and ensure staff are equipped to deliver high-quality care at all times. Together, WNSW PHN and MediCoach help RACHs document and implement local after-hours options, so care continues seamlessly beyond business hours.

We understand the complexity of providing round-the-clock care, especially in after hours scenarios. WNSW PHN supports RACHs with resources including the after hours plan template, used to document your facility’s after hours care plans.

 

RACHs in Western and Far West NSW can access after-hours support through Healthdirect’s GP Helpline and Virtual Care Clinic.

VirtualADULTS supports patients who may otherwise require Emergency Department (ED) care, offering a virtual alternative via Healthdirect.

Feature VirtualADULTS After Hours GP Support (RACH)
Purpose Alternative pathway to Emergency Department (ED) care via virtual access Supports RACH residents who need care after hours but do not require ED
When to use Patient is unwell enough for ED but may be safely managed virtually (exclusion criteria apply) Resident’s condition cannot wait for their regular GP but is not severe enough for ED
