CKD, Diabetes and CVD – make the link

Chronic Kidney Disease (CKD) affects approximately 1 in 7 Adult Australians, resulting in significant morbidity and mortality, and placing a large and increasing burden on the healthcare system. CKD is a stronger risk factor for future coronary events and all cause mortality than diabetes. Further to this, CKD rarely occurs in isolation, significant chronic kidney disease risk factors include diabetes and high blood pressure, so making the link and managing these conditions is important. The growing emphasis on patient centered care and the multidisciplinary team approach that primary care offers has increased the appreciation of the importance to overall wellbeing and clinical outcomes in CKD, Diabetes and CVD.
